Vehicle Make: KTM

Vehicle Model: 390 ADVENTURE R

Model Year: 2026

Summary

1. The Turn Signal Malfunction indicator is randomly triggered. Observation indicates this occurs when the ambient temperature is under 60 degrees. When the error code is displayed, the turn signals are non-functional. 2. The first time this occurred I was several miles from home at dusk. I had to turn onto and off of a highway without turn signals. At that time of day hand signals are not easily seen. This is dangerous. 3. This is the second bike my dealer has recently sold with this issue. The dealer left my bike in an unheated area over night and were able to duplicate the fault the next morning. 4. An incident has been filed with the Manufacturer but they do not have a repair currently. They were aware of the issue. 5. A warning light appears on the TFT dash when the fault is detected.

Manufacturer Name: Ford Motor Company

Vehicle Make: FORD

Vehicle Model: BRONCO SPORT

Model Year: 2022

Summary

February 26, 2026, Concern: Ford Recall 25S76 / NHTSA 25V467 – No remedy available. My vehicle previously had an earlier Ford recall for fuel leakage and fire risk. The dealer installed a drain tube as an interim fix about 18 months ago. Ford has now issued a new recall 25S76 / NHTSA Recall 25V467 August 2025 for a similar hazard involving cracked fuel injectors that can cause fuel to accumulate on the engine. Ford states that parts are not available and has provided no timeline for a permanent repair. Because this is the second fire risk recall for a related issue and the prior fix did not resolve the underlying problem, I am concerned that the manufacturer has not provided a timely or effective remedy. I am requesting that NHTSA review this delay and include my case in its monitoring of this recall.

Summary

The uconnect 4 system that controls everything on the vehicle will randomly reboot while being driven, disabling safety systems like reverse cameras during the reboot, and leaving the UI for uconnect incomplete after restart is completed so vehicle settings are not available until vehicle has been turned off for over 10 minutes. I believe that this is due to bugged wifi drivers since it seems to reboot when it tries to connect to android auto. I have video of a reboot showing my truck in reverse for exponentially longer than the required two seconds for the camera to come on but it is too large to upload here.

Manufacturer Name: Kia America, Inc.

Vehicle Make: KIA

Vehicle Model: SORENTO

Model Year: 2018

Summary

While on the road driving at 35 MPH, the vehicle started to "bog" and losing power and/or acceleration. Had to immediately drive off the road as there were several vehicles behind. As soon as driving off the road, the vehicle died down and would not start again. The check engine light came on. After several minutes the vehicle started again and drove to a secure area to call the tow truck. If the vehicle died down while in traffic, it would have caused an accident involving several vehicles. If it happened on the freeway, it would have caused a "major accident". The vehicle was towed to the vehicle dealership. It was determined by the vehicle dealership that the "high pressure fuel pump" was faulty and had to be replaced. It took four (4) months for the replacement part to arrive. Another similar dealer said that there were "55 other vehicle owners" waiting on the same part nationwide. It must be an "inherent problem" with the high pressure fuel pump. After replacing the high pressure fuel pump, the vehicle was operating normally.

Manufacturer Name: Tesla, Inc.

Vehicle Make: TESLA

Vehicle Model: MODEL 3

Model Year: 2024

Summary

I continue to experience phantom "forward collision avoidance" warnings. Tesla has looked at this several times, even once charging me $60 to clean the cameras (should be under warranty in my opinion). Latest response to problem: known issue, no known hardware cause. In other words live with it. They even suggested buying FSD package at $1200/yr as a solution! One more than one occasion the TACC and Autosteer disengaged with traffic behind me. Very scary.

Vehicle Model: ASCENDER

Model Year: 2007

Summary

The contact owns a 2007 Isuzu Ascender. The contact stated that while the vehicle was parked and turned off, the driver’s side window unexpectedly rolled down. There were no warning lights illuminated. Upon entering the vehicle, the contact noticed an abnormally strong smoky odor inside the vehicle. After inspecting the vehicle, the contact discovered that the driver’s door master window switch module had melted and was inoperable. The contact ordered a replacement part for the failure. The dealer was contacted. The vehicle was not diagnosed or repaired. The dealer informed the contact that the vehicle could not be repaired because the door master window switch module was an older module. The manufacturer was made aware of the failure. The approximate failure mileage was 94,000.

Manufacturer Name: Chrysler (FCA US, LLC)

Vehicle Make: CHRYSLER

Vehicle Model: PACIFICA

Model Year: 2022

Summary

The car randomly gives a warning on the dashboard in red, with red turtle symbol, to pull over and stop safely. A lot of the time we will be on the highway and can't pull over safely. It happens at any speed. 25, 80, 65, etc. It's been happening since we got the vehicle in 2023 and no dealership understands what the issue is even though there are clearly recall repairs for other same make and model of other years surrounding this 2022 model. They keep saying its Keysense but its not locked for speed and that doesnt make sense because it doesnt happen at a high speed only, it happens randomly even in town going slow. They've done the "software update" a fee times, but it's clearly the same problem as the others with the wiring harness. That part needs to be replaced because the patch software fix isn't a solution and its definitely not a safe solution at that.

Manufacturer Name: Subaru of America, Inc.

Vehicle Make: SUBARU

Vehicle Model: CROSSTREK

Model Year: 2026

Summary

Not an 'incident' but seat belt does not adjust low enough for an average short woman (5'1") without draping across lower neck. I have adjusted the slide on side panel to lowest point but seat belt still cuts across neck (not collar bone area). This is standard for all similar cars and I feel a seatbelt across one's neck is a safety concern that needs to be addressed. Adjustments of seat and/or steering wheel make little to no difference on seatbelt across lower neck area. Raising seat makes it difficult to get into vehicle, as it also moves the seat forward when it is raised (and seat position is manual adjustment, not electric anyway - and doesn't make seatbelt much lower - still neck area)....

Manufacturer Name: Honda (American Honda Motor Co.)

Vehicle Make: HONDA

Vehicle Model: ACCORD

Model Year: 2019

Summary

Approximate date (around January 2026), location (e.g., Newburgh, NY area), and circumstances (e.g., during normal driving). No crash/injury mentioned in my case, but note if it stranded I, required towing, or felt unsafe (e.g., potential for sudden power loss). While driving, all dashboard warning lights came on suddenly. The vehicle experienced rough running/misfires. Dealer diagnosed low coolant, pressure tested the system, and confirmed coolant intrusion into cylinders 2 and 3 due to head gasket failure. This required full head gasket replacement (parts/labor ~$4,567 total before any goodwill). No external leaks noted initially. Concerned this could cause engine stall/overheat while driving, posing a safety risk.” Dashboard lights, possible misfire codes (if any DTCs like P0302/P0303 were pulled), low coolant without visible leaks. dealer diagnosis (Morehead Honda? from Eric Koller), repair done/recommended, cost, and that American Honda provided 35% goodwill (shows it’s out-of-warranty but recognized issue) Only 91000 miles on the vehicle.
