Summary
The morning after a successful level 2 charge (from 35% to 80%), the ICCU fuse blew, resulting in no power to the car. I had to have it towed to have it fixed (both the fuse and ICCU were replaced under warranty). The car had 20,013 miles when the fuse blew and I had both safety recalls (software updates) addressed when they came out. I thought the updates were to prevent this from happening. Fortunately I was in my garage and not on the road; however, just the evening before, I was on I-95 for over an hour and glad it did not occur then. So now I'm fearful that it could happen again when far from home.

Component or system that malfunctioned: The braking system on my 2022 Toyota Highlander Hybrid, specifically the transition between regenerative braking and hydraulic friction braking, appears to malfunction intermittently. During braking, the vehicle sometimes lurches or surges forward instead of slowing smoothly, especially at low speeds or when braking downhill. The vehicle is available for inspection upon request. How safety was put at risk: The unexpected forward surge while braking increases stopping distance and creates uncertainty about braking performance, particularly in traffic or parking situations. This creates a risk of rear-end or low-speed collisions and reduces my confidence in the braking system. Problem reproduced or confirmed by dealer or service center: The vehicle was inspected by a Toyota dealership, and a Technical Service Bulletin (TSB) software update intended to address braking behavior was performed at approximately 50,000 miles. However, the braking problem continues unchanged after the repair. I had to pay the dealer to perform this TSB update. The dealer was initially reluctant to apply the TSB and did not associate my braking complaint with the known issue. I had to research the TSB myself and request that it be applied. The invoice notes that the dealer reproduced the issue. Vehicle or component inspected by manufacturer or others: The vehicle has been inspected by a Toyota dealership service department. It has not been inspected by police or insurance representatives, as no accident has occurred. Warning lamps or prior symptoms: No warning lights or messages appear when the problem occurs. The issue happens intermittently without warning and has occurred since the vehicle was new. The condition appears to be getting progressively worse. The issue seems more likely to occur when braking while the hybrid battery is near or at full charge, suggesting a possible problem in the transition between regenerative and friction braking.

The contact stated that there was a round dot on the front passenger’s side tire. Upon closer inspection, the tire displayed a crack and a large groove along the sidewall, and the sidewall lettering was missing. The other three tires were manufactured by Continental but were a different tire line and showed no visible damage. The vehicle was taken to a tire center for inspection, where it was determined that the bead was broken and that the tire had been driven while flat. The vehicle was then taken to a local dealer, who informed the contact that two of the tires needed to be replaced. The tires were not replaced. The dealer confirmed that there was no warranty or recall coverage available for the failure. The manufacturer was not contacted. The failure mileage was 17,463.

Summary
My wife and child drove the car to the grocery store and parked the car l. When she returned to leave , the car would not start and flashed a warning with key fob not detected and some kind of security light warning. The instrument cluster told her to place the key in left console cup holder to crank the vehicle . This did not work either .I drove the second key fob to her to determine if it was a key fob issue and the 2 nd key did not work as well. Called dealership and service requested the car being towed. Tow truck didn’t feel comfortable with the removing the vehicle as it was sitting due to space in the parking lot. They dispatched a small tow truck , but in the meantime I disconnected the battery cable and waited about 15 min. The car cranked and I immediately drove it to dealership. There was no code shown at the dealership and this was the first time they have had dealt with this issue. The mechanic drove the car the next day and the same situation happened again , except this time the key fob worked after several failed attempts . I feel that this is a safety issue because the car failed to operate and had no manual ability to operate without the electronic system functioning properly and this could have happened anyway and stranded my wife and child . A few months before in an unrelated incident , the power trunk malfunctioned and had to be reprogrammed . It would not operate except manually . Car is currently at dealership still being evaluated .
