Summary

Dear NHTSA Office of Defects Investigation, I am submitting this complaint to formally report a transmission failure involving a vehicle equipped with the 10R80 automatic transmission, which I believe represents a serious safety concern and may be part of a larger pattern of premature failures. The transmission began showing signs of failure at approximately 52,000 miles. Early symptoms included hesitation, harsh or delayed shifting, inconsistent engagement, and warning signs related to drivetrain or transmission performance. I sought service and diagnosis, but despite many similar failures being reported by other owners, no corrective action or goodwill repair was offered by the manufacturer or dealership. I was quoted approximately $9,000 by the dealership for a full transmission replacement. Due to the high cost, I continued operating the vehicle while monitoring the issue. Over time, the condition worsened significantly. As the transmission continued to fail, the vehicle would sometimes move forward and then suddenly stall or lose drive power while in traffic. This occurred while my family was in the vehicle and created a major safety concern, as the truck could unexpectedly lose movement in active traffic lanes, putting us at risk of a collision and leaving us unable to safely accelerate or clear intersections. On January 30, 2026, the transmission failed completely. The vehicle no longer moved when placed in gear, leaving it inoperable and unsafe to drive. Because assistance was denied and no recall or corrective action was available, I was forced to repair the vehicle at an independent repair shop at my own expense. The repair cost is approximately $7,000 out of pocket. Odometer Fraud. The contact purchased a 2017 Chevrolet Tahoe on February 21, 2026. The contact used another vehicle as a trade-in to purchase the vehicle. The contact stated that the odometer was recorded at 36,688 at the time of the sale. The contact was provided with an Experian report, which indicated that the odometer reading was last recorded at 35,900. The contact purchased the Carfax report the following day, which indicated that on March 25, 2025, the last odometer reading was 109,967. The sales representative was contacted and informed of the discrepancy, and was informed that the issue would be resolved. The contact was informed that the trade would be voided.

Summary

The contact owns a 2022 Audi E-Tron. The contact stated that while driving at various speeds, the vehicle independently switched to SAFE Mode and failed to properly accelerate while depressing the accelerator pedal. Additionally, the mileage range was reduced, and the battery recharging time increased. The failure started after the vehicle was repaired under the NHTSA Campaign Number: 23V867000 (ELECTRICAL SYSTEM). The vehicle was taken to the local dealer, but no failure was found with the vehicle. The vehicle was later taken to an independent mechanic who reported that the amperage of the battery had been reduced from 400 amps to 130 amps. The dealer was contacted and confirmed that the part was not yet available for the repair. The manufacturer was made aware of the issue and confirmed that the part was not yet available. I have been having to constantly replace key fobe batteries since I bought it in May of 2025 needing jumps although auto zone said it shows no issues with battery ? Often no key detection I just changed the batteries at dealership but everyday just about I need a jump it’s frustrating, dealership was trying to sell me a battery? My antifreeze is low Nissan said it’s the overflow because it looks empty auto zone also said I needed it. Fluids should have been topped off at time of purchase. And was told that stuff lasts forever help!

Summary

Randomly, and without warning, the instruments, gauges, and displays will switch into a language I do not speak or understand (Italian). Coinciding with the switch to Italian language, cluster and radio display settings revert back to default settings, auto door locks and paddle shifter becomes inoperative (with no menu to enable), pedestrian emergency brake (peb) lamp or forward collision warning (fcw) lamp turns on with no way to disable, units of measurement including speed, temperature, etc. all change, and many other random settings change as well. ***This is clearly a dangerous situation because of all the functions and gauges that change, and the fact it is difficult if not impossible to recover from this due to the language of the setting menus, and the complexity of discovering and navigating through all these changes. When this happens, even if one is able to wade through all the settings menus and change setting back, some items cannot be reset (like the FCA off lamp). I have had this to the dealership 4 times to be fixed without success. Because this is dangerous (for settings/gauges/units to no longer be useful while driving), and it occurs randomly.

Summary

The vehicle has suffered four (4) catastrophic engine failures due to "coolant intrusion" into the engine cylinders, a known defect in this model year. Despite the vehicle’s VIN showing that Ford Recall 17S09 (NHTSA 17V209) was previously "repaired" by a previous owner, the engine continues to overheat, crack the cylinder head, and fail. The cooling system and engine block are currently available for inspection. SAFETY RISK: During multiple failures, the vehicle experienced a sudden loss of motive power and stalled in active traffic without sufficient warning. This creates an immediate risk of a high-speed rear-end collision. Additionally, the overheating creates a documented fire risk under the hood, as coolant or oil can leak onto hot engine components. CONFIRMATION: The failure has been reproduced and confirmed by both a major national used car retailer’s service department and an independent certified mechanical service center. The independent shop confirmed that the engine block has failed due to coolant entering the combustion chamber. SYMPTOMS: Prior to the most recent failure, the "Coolant Over Temperature" warning lamp appeared briefly before the engine lost power. These symptoms first appeared within 30 days of vehicle purchase and have recurred through four separate replacement engines, proving the manufacturer's recall "remedy" is ineffective at preventing catastrophic engine failure.
