Summary

My 2022 Ioniq5 left me stranded late at night in a freezing rainstorm in March of 2024 when I was 75 miles from home at it failed to charge both D/C fast charging and A/C level 2 charging, despite my trying several different chargers. The car gave me the dreaded "Check E/V System" warning when it failed to charge. It turned out to be the notorious ICCU but also the AC inverter, which had to be replaced. It took 12 days. The dealership's initial diagnosis was something draining the 12-volt battery, so I asked if they also replaced that. They said no because it had tested fine, but 9 days after I picked up the car, it did not start because it turned out the 12-volt battery was dead, and my dealership spent 2 days getting Hyundai to approve replacement of the 12-vold under warranty. Last month, 17 January 2026, my Ioniq5 again left me stranded. This time 120 miles from home in a snowstorm. It D/C charged for 10 minutes before the charging stopped and it gave me the dreaded “Check E/V System” warning. I tried two nearby Tesla Superchargers with my Hyundai issued adapter, and they both failed. I had just enough charge to make it to a dealership 110 miles from my home, where the vehicle stayed for nearly a month while they diagnosed and repaired it by replacing the AC inverter (note that had been replaced the last time) and also the AC compressor. The ICCU failures are well-known and well-documented, but the AC inverters that failed on me twice do not seem to be as common and should probably be investigated. In both cases I was stranded on Saturdays when no dealership service departments were open until the following Monday, so I had to pay for a motel overnight, which Hyundai refused to reimburse because I was not at least 150 miles from home.

Summary

I am reporting a manufacturing defect involving the dashboard on my 2022 Audi A6. The dashboard has begun bubbling and warping, particularly near the area close to the windshield. This defect appears to be common among Prestige‑trim Audi A6 vehicles, as other owners have reported similar dashboard warping. The warping causes the dashboard material to rise upward, which reflects additional glare and partially obstructs visibility through the lower portion of the windshield, especially in certain lighting conditions. This reduces my ability to clearly see hazards low on the road ahead, creating a potential safety risk. Additional Details: - The vehicle has not been modified. -The issue has progressively worsened over time. - The defect appears to affect normal forward visibility and could distract the driver. Audi has not issued a recall for this known problem, despite multiple similar complaints from other owners (Effected Models per online research: ETron, R8, A6, A7, A8, Q7, Q8). I am asking NHTSA to investigate this dashboard defect as a potential manufacturing defect and safety hazard due to obstructed visibility and distracted driving caused by glare/reflection from the warped dashboard surface.

Summary

I am filing a safety complaint regarding water intrusion into the rear tail lamp assembly of my 3-year-old Tesla Model 3. The rear tail light has progressed from minor condensation to active water accumulation and visible pooling inside the sealed housing. This is not light fogging — there is standing moisture inside the assembly. The vehicle has not been involved in any collision and there is no visible external damage to the lamp. I have already been pulled over, and drivers behind me have indicated that the affected tail light is not clearly visible. Reduced rear illumination creates a serious safety risk, particularly at night or in poor weather conditions. A malfunctioning tail lamp increases the likelihood of a rear-end collision. Rear lighting is a federally regulated safety component. Water intrusion into a sealed LED lighting assembly creates risk of electrical malfunction, corrosion, and sudden loss of rear illumination. The vehicle is currently out of warranty due to mileage; however, it is only three years old. A sealed LED tail lamp is not a wear-and-tear item and should not fail due to normal mileage accumulation. This appears to be premature sealing failure. I contacted Tesla Service regarding this issue. Their written response stated that because the vehicle is out of warranty due to mileage, any repairs would be at my expense. They did not address the safety concern or the presence of internal water accumulation. There are numerous reports from other Model 3 owners describing similar tail lamp water intrusion issues. I am requesting that NHTSA review whether water intrusion into sealed rear lighting assemblies constitutes a safety defect and whether further investigation is warranted.

Summary

The engine stalled while driving on the expressway. Myself and my daughter and her friend were in the car. The vehicle started right back up, and I was able to drive the car home (less than 2 miles away). The following day, my wife got an oil change, and went on the expressway, and the engine stalled again causing her to have to navigate the car, with no engine power, to the side of the highway and wait for myself and a tow truck. Both times were very dangerous. We had the vehicle towed to a mechanic, who said the crank shaft & rods and bearings for the engine look like they failed and seized up, causing the engine to complete lock up and stall, requiring a full engine replacement. Kia is refusing replacement saying we are not covered by the previous complaints and concerns, stating our vehicle is not covered specifically, stating that the 3.3L engines were not covered under the previous recall/extended warranty offered on engines in the same yr/make/model as our vehicle. The check engine light came on, and there was a slight knocking sound as the car was idling just before the engine stall, and as the engine stalled, the oil lamp and the battery light came on. This happened both times. The symptoms appeared just before complete engine failure. We get the vehicle serviced at Kia, and had the oil changed by Strickland Brothers the day of. We were concerned that the engine light came on due to potential of the oil needing to be changed. We do not have any electronic files or reports for this issue yet.

Summary

After learning of the recall for the lift gate module because of water intrusions of multiple points. The wiring harness to the lift gate module was serverly corroded and had to be replaced. The original fix, without the corrosion on the harness, was the place a piece of plastic over the module. That recall and fix did nothing to address the issues of multiple points of water intrusions. From body seems, the seal around the hatch, and around the tail light(s). There is more than one harness, and more than one module in the tire well. The multiple water leaks, that can and have affected an electrical component the was deemed as being a potential to start a fire, and none of the others in there are. There needs to be a recall on the water leaks, from their poor design! I have saw first hand how badly the harness they HAD to replace, along with the lift gate module. Because of the very seriousness of the corrosion. Where they not only replaced both of them, but also had to relocate the lift gate module, because of the evidence of the amount on water that has been entering the cargo, and wheel well areas! À 11 year old vehicle, from the information I have read up on, many with this same production years are also having the issues of water intrusions! Hold them responsible, instead of letting them cut corners of just trying to put a umbrella over there recall! I don't have pictures now, or when I took it to the dealership originally for the recall. But I am willing to take pictures, as I'm still in the process of disassembly the interior to try and pin point all the water intrusions! The dealership I took it to for the recall work, said they are "not aware of any water intrusions on this vehicle". That's a big problem!!

Dear NHTSA Investigators, I am writing to formally report what I believe is a significant safety defect in certain 2007–2011 Toyota Camry vehicles, specifically related to excessive engine oil consumption in models equipped with the 2AZ-FE 2.4L four-cylinder engine (primarily 2007–2009, with some reports extending to later years). This issue causes the engine to burn oil at an abnormally high rate—often 1 quart or more every few hundred to 1,200 miles—without any visible external leaks. Owners frequently report no adequate warning from the dashboard oil light or other indicators before the oil level drops critically low. If the engine runs out of oil while driving, it can lead to rapid overheating, internal component damage (such as scored bearings or cylinder walls), and sudden engine seizure or catastrophic failure. Such a failure while the vehicle is in motion poses a serious safety risk, including: potential for crashes, including rear-end collisions if the vehicle stops abruptly without warning, or veering into other lanes/obstacles. Sudden loss of engine power. Increased likelihood of loss of control etc. This problem has been widely documented through thousands of owner complaints (e.g., hundreds on sites like CarComplaints.com for the 2007 model alone), NHTSA ODI reports, and Toyota's own Technical Service Bulletins (TSBs) and Warranty Enhancement Programs (such as ZE7 for certain models). While Toyota addressed it via a warranty extension (not a full recall) involving piston/ring replacements for qualifying vehicles after an oil consumption test, many owners—particularly those outside the original coverage period or with higher mileage—have faced expensive engine repairs or replacements out of pocket. The lack of a mandatory recall means the issue persists in many vehicles still on the road. I am concerned that this defect creates an unreasonable risk to drivers, passengers, and other road users. Summary

Purchased car new. Problems with brake system started at 6k mls. Problems got progressively worse to 29k mls where now rotors are being replaced due to “extreme warping” with pads, calibers, and everything else deemed fine. On brief internet search it was apparent that this rotor warping at low milage is a common problem. Warped rotors are a safety hazard to the driver, their passengers, and everyone else on the road with them. Please investigate.

Cvt Transmission failed While driving transmission popped and no forward drive leaving us in traffic unable to move in forward. Had to get out and push the car to the side of the road. No warning lights no warning signs. Just immediate loss of forward drive. Engine revs up but no movement. Dangerously leaving us in harms way. Haven't had the vehicle inspected by nissan yet but will be doing so in the next day or so. Vehicle is and will be Available for inspection or assessment at any time.
