Description of Incident: On February 15, 2026, my 2017 Chrysler Pacifica Hybrid experienced three distinct and severe safety system failures involving the electrical system, power seats, and the power sliding door. Power Sliding Door Sensor Failure: While my minor son was entering the vehicle, the power sliding door closed on his leg. The pinch sensor failed to detect the obstruction and did not reverse. This resulted in physical bruising and marks on the limb (photographic evidence maintained). Unintended Passenger Seat Motion: While the vehicle was stationary, the front passenger power seat moved forward to its maximum limit without user input. I was seated in the chair at the time and was pinned against the dashboard before I could intervene. Electrical Shock: My minor daughter received a perceptible electric shock upon touching the exterior door-handle button to open the sliding door. Components Involved: Electrical System Seats (Power) Sliding Door (Power) Injuries: Yes (Soft tissue bruising/marks to minor’s leg). Risk to Safety: Had the driver's seat moved forward without input while the vehicle was in motion, this absolutely could result in loss of control of the vehicle and an accident. Had the sliding side door continued to close on my son's leg, it could have caused severe injury. Had the electrical shock been worse, it could have resulted in burn or injury. The vehicle is available for inspection. It has not yet been inspected by the manufacturer, police, or insurance. The issues have not repeated at this point, although the vehicle was not used the last 12 days. I do not have confidence in its electrical components. There were no warning lamps, messages, or other indications of any problem with the vehicle.

The check engine light came on and the engine lowered performance because the intercooler froze and cracked. Despite there being a service update to stop that from happening, we were never informed of the update from Chevy, only once it had happened did they tell us about the update and included it in the repair cost.

[XXX] Message says warning "check ev system". they then sent a letter saying it had to go to the dealer, or the car would need A tow potentially. I can't get an appointment as they are way too busy to do anything other than tell me they will get back to me, repeatedly stating they will do so in one business day. it has been a week. My wife is scared to drive the car. I think it is bogus as no recall has been issued. I have had the car for 19 months with no such warning. Two days after it shows up on the screen in the car i receive the letter. This is fishy. it is either a recall, or it isn't. if it is why is it not in your data base? Summary
2018 Pacifica with 3.6L engine. Blown head gasket at 69k miles with coolant leaking into cylinder 5. The engine is very hesitant upon acceleration and can be dangerous when pulling out into traffic. This is an all too common issue and needs to be addressed by Chrysler. Engines should not be failing this at 69k. I change oil every 4k miles which is way ahead of the recommended schedule by Chrysler. Coolant was done by the dealer after radiator replacement for a radiator leak at 52k miles.

Summary
Following engine replacement per NHTSA for 6.2l GMC engine. I am submitting a safety complaint regarding my 2022 GMC Sierra 1500 Denali (refresh body style). The vehicle has experienced repeated failures of driver-assist features and safety system warnings, including: Dash messages: “Lane Keeping Assist System Unavailable” and “Service Safety Restraint System.” When the fault occurs, driver-assist features including Adaptive Cruise Control become inoperative. The issue has recurred multiple times, including after repairs. After one event, the truck sat parked approximately 10 days and the battery discharged to failure and required replacement. A dealership visit documented DTC U3000 (“Internal Electronic Failure”) in the front view camera module. The dealer replaced/programmed/calibrated the front camera on 02/09/2026 at approximately 58,405 miles (invoice/RO #285795). The symptoms have recurred despite prior resets and this repair. I am concerned about loss of driver-assist functions while driving and the presence of a safety restraint system warning. Please investigate whether there is a defect related to the front camera/driver-assist systems and/or electrical module behavior that can cause repeat faults and battery drain on this model.
