Technical Incident Report: Electrical Component Failure Reference: Technical Service Bulletin NTB19-004 Part Number: 873D6-5AA6A (Control Unit/Actuator) Incident Overview On 2/16/26, the vehicle was being operated by the primary driver when a suspected electrical failure occurred. Upon subsequent inspection, the 873D6-5AA6A control unit and its associated wiring harness were found to be severely compromised by thermal damage. Detailed Observations: Thermal Damage: The control unit housing showed significant melting. Residual smoke was present during the inspection, indicating a high-temperature event caused by a short circuit or component failure. Harness Integrity: The wiring harness insulation was melted and charred. Due to the extreme heat, the plastic connector had become brittle and lost all structural integrity. Component Failure: During the attempt to disconnect the unit for safety inspection, the connector shattered, leaving the internal wiring of the harness exposed. Documentation: High-resolution photographs have been captured to document the state of the melted housing, the shattered connector, and the exposed wiring.

Summary
We were notified by Toyota that it was unsafe to transport our children in the second row seats in November 2025 and the vehicle was under a recall. This puts our children at risk. A letter from NHTSA stated Toyota would have a remedy by 12/6. The manufacturer (Toyota) confirmed this recall. The seat always shook but we assumed it was the carseats shaking, but, upon removing them to take the car to the dealer, we were still hearing the shaking even without the carseats. Given that Toyota has not indicated a solution date for this and the problem was discovered in October 2025 I believe the manufacturer has failed or is unable to remedy this safety recall for our vehicle in a timely manner and would like to officially log a complaint with NHTSA.

Summary
In several situations, I have lost partial or complete power steering ability. Once, while driving, I recieved a SERVICE ESC message and steering became quite difficult. Another time, power steering was completely gone, making steering impossible, although every other aspect of the vehicle operated normally. In both instances, there was no clear issue and after some time with the vehicle parked and off, it would return to normal. Possible loss of power steering unexpectedly is a serious concern at this point.

The vehicle previously received a manufacturer dash wiring recall repair. After the recall repair was completed, the vehicle began experiencing intermittent and cascading electrical and communication failures across multiple integrated systems. Affected systems include Apple CarPlay, the DVD system, in-vehicle camera systems, CabinWatch, and repeated “MOST network unavailable” error messages. These failures occur intermittently and result in multiple systems disabling simultaneously. The vehicle was inspected by a Honda dealership, which confirmed the issue involves wiring within the dash harness. The dealership stated the affected wiring is not sold or serviceable separately and that correction would require replacement of the entire dash harness, but that this portion of the harness is not included in the original recall coverage. Because these systems share common power and communication pathways, failures cascade across multiple modules. When the camera system fails, rear view camera functionality and related warnings are unavailable while backing up, reducing the driver’s ability to detect obstacles, pedestrians, or children behind the vehicle. When CabinWatch is unavailable, the driver is unable to visually monitor children seated in the rear rows while driving, which affects situational awareness and the ability to respond quickly to unsafe conditions inside the vehicle. These failures also impact driver information systems and create intermittent loss of critical visual feedback, increasing driver distraction as systems drop out unexpectedly while the vehicle is in operation. The condition began after the recall repair and appears to be related to or a continuation of the original recall defect. The vehicle remains available for inspection, and the condition has been confirmed by a manufacturer-authorized dealer.

Driver side front window auto rolled back up after pressing the button to once to roll it down. This happened while driving while the right hand was on the steering wheel.
