My 2023 Gladiator currently has 36,000 miles and is experiencing severe front-end vibration commonly referred to as “death wobble.” The issue occurs at approximately 60 mph when hitting a bump on the highway, causing the steering wheel and front end to shake violently. I have scheduled an appointment with an authorized Jeep dealer for inspection. This condition has nearly caused an accident, as I was traveling on a busy highway and had to slow down abruptly and pull over to regain control. While on the highway my car lost power, gradually slowing down to a stall. I was able to safely park on the side of the road. I got the car to a nearby dealership that kept it for 10 days. The dealership said that the problem was code P2118 (Throttle Actuator Control Motor Current Range/Performance) that the air "flap" was frozen stuck shut and the engine was disabled on the freeway in order to prevent damage. The resolution was "waited for it to thaw and cleared the code". The dealer has a service bulletin and a software update but it did not resolve the safety issue. I've noticed hesitation when accelerating the car from a dead stop as well, which started last year.

Oil consumption is excessive causing the vehicle to stall out, exhaust smoke and loose power. Upon several recommendations from the service shop, money has been dumped into repairs that has not resolved the issue. Now, I am adding oil every other week with no oil leaks. I am afraid I will keep dumping unnecessary money into this issue; however, I am more concerned I may eventually stall out on the interstate, highway, or unsafe area.

The rearview camera on my 2014 Ford Fusion has been malfunctioning for several years. When the vehicle is placed in reverse, the camera image frequently does not display, displays intermittently, or fails while already in use. When the system does activate, the image is sometimes mirrored or reversed, making it difficult or unsafe to interpret the surroundings behind the vehicle. Overall, the camera functions correctly in approximately 1 out of 10 attempts, and cannot be relied upon for safe operation. This failure is unpredictable and occurs without warning, creating a safety risk due to the inability to consistently see pedestrians, children, or obstacles behind the vehicle. The vehicle was inspected by a Ford dealership for this issue; however, no remedy was provided because the vehicle was outside the warranty period. I am aware that Ford has issued recalls and extended coverage for rearview camera failures on later model year Ford Fusion vehicles (2015 and newer) exhibiting similar symptoms. My vehicle demonstrates the same defect behavior, despite being excluded based solely on model year. Although rearview cameras were optional equipment in 2014, this vehicle was factory-equipped with a camera, and the failure constitutes a safety-related defect rather than normal wear. I am requesting that NHTSA review whether this defect affects earlier model years, including the 2014 Ford Fusion.

On January 28, 2026, the vehicle (a Tesla Model Y) was operating with the Full Self-Driving (FSD) system actively engaged in normal highway traffic conditions. Without any prior warning messages, chimes, or system alerts, the FSD system abruptly disengaged approximately 1 second before the vehicle steered into a concrete road divider. Component failed: The Tesla Full Self-Driving (FSD) software and underlying perception/control systems failed to maintain the lane and abruptly handed over control with insufficient time to react. The vehicle and its data are available for inspection. Safety risk: The sudden, sub-second disengagement provided the driver with zero time to react, take manual control, or perform an evasive maneuver. This system failure directly resulted in a high-speed collision with a road divider, causing significant vehicle damage and physical injuries to the driver that required medical treatment. Inspection/Confirmation: The vehicle is currently subject to an open insurance claim and has been evaluated for repair. Warnings: There were no warning lamps, messages, or requests for the driver to take over prior to the immediate 1-second window before the crash. Evidence: I have downloaded and preserved the complete Event Data Record (EDR) and the multi-angle dashcam video files. The EDR data mathematically documents the sub-second disengagement, and the video files confirm normal traffic flow with no external hazards forcing the collision. I can provide the EDR PDF and video files to NHTSA investigators immediately upon request.

Summary
Driving approx 40mph on the road and a loud thump sound followed by grinding from rear end started. Came to stop in the road and when the gas pedal was pushed the grinding sound continued. The truck was pushed off the side of the road and had to be towed to the dealership since January 4th. It has been at the dealership with no estimate of time for repair from the dealership so far and we just keep being told there is nothing they can do. The dealership states that the parts are on back order and we are in a line of several other trucks with the same problem.

I am reporting a potential safety defect involving the driver seat lumbar support mechanism found in multiple vehicle models across the industry. The lumbar support feature applies excessive, uneven, or unstable pressure to the driver’s lower back during normal driving conditions. This occurs even when the feature is set to its lowest or neutral position. The defect presents in the following ways: •The lumbar support protrudes too far forward, forcing the spine into an unnatural posture. •The mechanism shifts or changes pressure during , creating sudden discomfort or pain. •Extended use results in lower back strain, muscle spasms, or nerve irritation. Safety Risk This issue poses a significant safety hazard because it directly affects the driver’s ability to maintain proper posture, comfort, and control of the vehicle. This pain is not noticeable until you step out of the vehicle and it difficult to trace that to the lumber support button as most people do not observed when it is on. The defect can: •Reduce reaction time due to discomfort or restricted movement •Increase the risk of long term musculoskeletal injury •Create fatigue or numbness that compromises safe operation Because this feature is present in many vehicles and consistently causes injury, the problem appears to be systemic and design related, not limited to a single manufacturer. Frequency of the Issue •Occurs: (e.g., every drive / frequent / intermittent) •First noticed on: (2020) •Conditions: (e.g., highway driving, long trips, stop and go traffic) Additional Notes •No aftermarket modifications have been made to the seat. •It may not be limited to Toyota vehicle only. I guess any vehicle driver seat that has lumber support is a suspect. •The issue persists regardless of seat height, tilt, or recline adjustments. •The problem appears inherent to the lumbar mechanism’s design rather than wear and tear. •The defect affects driver comfort, posture, and safe vehicle operation.
