Summary

I bought this vehicle used on 04/23/2024 with 18,596 miles on the odometer. When I was inspecting the vehicle prior to delivery at the dealership, I noticed a crack low on the windshield. The dealership replace the windshield prior to my taking delivery of the vehicle. In the spring of 2025 I had a crack develop in the windshield and had to have it replaced. Another crack has since developed (it is now 2/25/26), and I will, once again, have to have the windshield replaced. I have not driven on any freeways recently, so no high-speed travel. I have been driving for over 40 years and have never had this frequency of windshield failures. I have become aware that this issue is discussed by numerous newer model year (2021, 2022, 2023, etc) Toyota Highlander owners. Some have replaced their windshield multiple times like me. It would appear that there may be an issue in the design/aerodynamics of the front/windshield or a defect in the structure of the body that supports the windshield, that leads to this increased frailty of the windshield. I would very much like to see the NHTSA look into this issue and to force Toyota to look carefully at the high number of windshield failure incidents in these vehicles.

Manufacturer Name: General Motors, LLC

Vehicle Make: CHEVROLET

Vehicle Model: EQUINOX EV

Model Year: 2025

Summary

The system that failed is the headlight design. I was driving home this evening in a moderate snowstorm, and the snow accumulated and completely blocked the headlights. It was nearly impossible to see the road in the dark without the lights providing adequate lighting. This put the safety of my wife and me (the passengers), and all other cars on the road at risk. This can't easily be reproduced without a snowstorm, particularly wet snow that catches and does not just brush off, but instead accumulates in the recessed light design.

Summary

Detailed Description of the Incident: I am reporting a recurring powertrain safety issue regarding the 8-speed automatic transmission (UA80-series) in my vehicle. [Symptoms] During cold-start conditions (outside temperatures below 30°F), the vehicle consistently experiences a violent jerk/shock and an audible clicking noise during the first 2nd-to-3rd gear transition. This physical impact is severe enough to be felt directly by the driver through the accelerator pedal. The issue is highly predictable, occurring once after every "Cold Soak" period (6+ hours of sitting). [Dealership Failure] I presented the vehicle to an authorized Toyota dealership for diagnosis. Although the conditions for the issue were met, the technician stated they "could not feel or verify" the shock that I, as the primary driver, clearly experience. By failing to acknowledge a physical powertrain shock, the dealership is dismissing a potential mechanical failure as "normal operation" simply because no Diagnostic Trouble Codes (DTCs) are triggered. [Safety Risk] The failure to verify this symptom does not eliminate the safety risk. This unexpected shift shock causes a momentary hesitation in power delivery. If this occurs while merging into traffic or navigating an intersection immediately after a cold start, it creates an unpredictable vehicle response, significantly increasing the risk of a rear-end collision. [Request] I request NHTSA to investigate this specific 2-3 shift shock in 2023-2024 Toyota Highlanders. Toyota’s reliance on electronic codes to diagnose physical transmission defects is leaving drivers at risk of sudden powertrain failure or accidents in cold climates.
