Summary
My 2022 Ford F150 wouldn’t start one morning. There was power and a lot of codes came up on the dash pod. I had it towed to the Ford dealer and they confirmed a no start situation. They replaced the under hood fuse box (warped from heat) and ultimately found a chaffed wiring harness with corroded and broken wires. It’s my opinion that a wiring harness is not a wearable item and this was caused by a friction situation. It’s my opinion that this could potentially be a safety issue if I was driving the vehicle and this happened it would be a total loss of power. The Ford dealer repaired the 2 wires and re-taped the harness.

Summary
Tesla decided to remove the safety unlock illuminated button from its front trunk (frunk) sometime in mid 2025. This can be a safety issue which was present in earlier models and against the Federal Motor Vehicle Safety Standard (FMVSS) 401, mandating an interior release mechanism for enclosed trunks to prevent entrapment. I contacted Tesla and they told me that my vehicle was made after Tesla decided to remove this safety feature.

Manufacturer Name: Subaru of America, Inc.
Vehicle Make: SUBARU
Vehicle Model: OUTBACK
Model Year: 2018
Summary
The STARLINK touchscreen display on my 2018 Subaru Outback has experienced progressive delamination — a separation of the screen's protective layer from the display panel — resulting in large, spreading opaque blotches that obscure significant portions of the screen. This is a known manufacturing defect that Subaru of America acknowledged through the issuance of a Technical Service Bulletin and an extended warranty for 2018 model year vehicles. That extended warranty has since expired, and Subaru has declined goodwill coverage despite the defect being documented by an authorized Subaru dealership (SOA Case No. 260106-1800881). The screen serves as the primary interface for the backup camera, navigation, and climate controls. Obscured visibility of this screen while driving creates a distraction hazard. I am requesting NHTSA investigate this defect and take appropriate action.

Manufacturer Name: General Motors, LLC

Vehicle Model: SIERRA 1500
Model Year: 2014
Summary
The vehicle experienced complete lifter failure on cylinder number 4 bending the intake valve push rod thus shutting down the cylinder. The vehicle has 167,000 miles and is well maintained. The vehicle gave no warning signs that there was a problem. Simply driving down the road, normal engine temperature, normal oil pressure, the check engine light comes on at the same time the engine begins to mis-fire. Pulling over to the side of the road, I could hear lifter noise from the right bank of the engine. The dealership (Ocean Crest Chevrolet/GMC Warrenton, Oregon) confirmed the damage. The mechanic and the service manager both state the damage is a direct result of the GM cylinder deactivation system, and every single one they have repaired has been cylinder number 4. They further stated that GM has recommended a lifter change when repairing the damage, in other words, the lifters that were installed when the engine was originally built are defective. This damage occurs on both the 5.3 and the 6.2 with cylinder deactivation, and according to the dealership, it begins with cylinder number 4. This problem has also been responsible for cam damage which results in total engine replacement/rebuild due to metal contaminating the oil.
