Summary

October, 2025: At around 1800 miles on the trailer we inspected one axle set (RR)of wheel bearings and brake backing plate/shoe assemblies. This was prior to a 1400 mile trip with the trailer. The trailing shoe was found to be cracked and had material coming off in chunks. Upon contacting and reporting this to the manufacturer they sent a replacement backing plate assembly, drum and wheel bearings. The planned trip was made with the old parts. The braking on the trailer wasn't the best but they did not fail. Skip ahead to 02-22-2026 and we took the brakes apart on the one we'd reported and noticed immediately that the friction material that had begun to come apart had done so even more. We then sent another message to manufacturer regarding this, them took the remaining 5 wheels and brake drums off the trailer. Several had either cracked or missing friction material. One assemble, LF, had been greased excessively and had grease in the brake actuation coil, shoes and drum. I again sent a message with pictures to the manufacturer. They got back to me pretty quickly and parts to replace all of the brake assemblies are being shipped. Our worry is that many more units have this same faulty brake friction material.

Summary

On 8/14/24 I had the rear springs replaced due to a broken spring. Now today 2/25/26 I was inform that I have a broken front spring. This vehicle is only 9 years old and we have had spring break in the front and back. This seems like a premature failure of a critical suspension component. Though fortunately nothing happened in the way a bodily injury the failure could very well lead to an inability to properly control the vehicle if the broken spring were to slip and crash is possible. For example a tire could be punctured at a high rate of speed. I may not be able to maintain control with the broken suspension component. The vehicle springs should last more than 7 years before failure. I have owned many vehicles over the years a lot older than this one and with more miles and have never experience spring failure.

Summary

Sudden loss of acceleration. This occurred while making a left turn across traffic from a stop. At approximately 7mph the car would not accelerate any further. I was luckily able to coast the vehicle into a center turn lane. Upon coming to a stop I was unable to get any acceleration. The vehicle was turned off and turned back on again and operated normally. There were no trouble codes displayed.

Summary

On Sunday, February 22, 2026, I was driving my 2013 Ford Flex with the driver’s seat heater activated. After a period of use, I began to notice an unusual electrical burning smell inside the vehicle. There were no dashboard warning lights or system alerts. I turned the seat heater off immediately once I detected the smell. I did not initially notice an injury at that time. The following day, I experienced significant pain and discovered burns on my buttocks in the area consistent with seat contact. I sought medical evaluation three days later and was diagnosed with second- to third-degree burns. Prescription treatment was provided. The vehicle has not yet been inspected for the cause of the overheating or odor. Based on the location and pattern of injury, and the timing of the electrical smell while the heated seat was activated, the burn appears to have resulted from a malfunction of the seat heating element.

Summary

PHEV sometimes fails to "start". That is, vehicle will not enter "Ready" mode when brake is depressed and Start button is pressed. Instead, 'Refuel Error', 'Stop Safely Now', 'Service AdvancTrac', 'PreCollision Assist Not Available', and several other warnings appear. This only occurs on the first 'start' of the day, when the EV battery has been recharged overnight, indoors. Outdoor use/charging does not have this effect. Concerned that something is overheating during indoor charging. Dealer has not been able to fix.

Summary

In the middle of a cold snowstorm the screen froze and started glitching. As I was trying to change the drive mode to all wheel drive the ESC off button triggered and ESC stayed off without AWD mode and screen was unresponsive. Had to pull over and turn car off and back on for screen to reset.

The engine cylinder head cracked and is leaking engine oil. This component is available for inspection at Liberty Mazda or by an independent certified mechanic upon request. The cracked cylinder head caused significant engine oil leakage, which created the following safety risks: Sudden loss of engine power or stalling while driving Smoke emission from the engine bay Potential fire hazard if leaking oil contacted hot engine components These conditions placed both the driver, passengers, and others on the road at risk. The cracked cylinder head and associated oil leak were diagnosed and confirmed by Liberty Mazda, the authorized dealership that performed prior repairs on this vehicle. The dealership documented the issue and provided a repair estimate of approximately $5,400. No independent service center has inspected the vehicle at this time. The vehicle and cylinder head have only been inspected by Liberty Mazda. Mazda Corporate has not physically inspected the vehicle, but all service records and documentation of prior repairs have been submitted to them during the warranty dispute process. There has been no inspection by police, insurance representatives, or other third parties May 2025: The vehicle intermittently struggled to accelerate. At that time, no dashboard warning lamps illuminated, and the dealership initially reported no issues. June 2025: Following the timing cover repair, the vehicle experienced engine oil leakage due to incorrect sealant installation, which was documented. February 10, 2026: The dealership repaired the oil pan seal and replaced the sealant, refilling engine oil. February 14–15, 2026: Approximately four to five days after the February 10 service, the cylinder head cracked, causing a significant oil leak. All symptoms and related service events are documented in Liberty Mazda’s service records.
