Summary

"CATASTROPHIC ENGINE FAILURE & INEFFECTIVE RECALL REMEDY (NHTSA 19V-204): While traveling at highway speeds, with my two children and boyfriend, the vehicle went completely black. Car loss complete power, going into a limp mode( 'Loss of Motive Power') The Check Engine Light began flashing before going solid. When I was able to pull to the shoulder, I tried to start the car and noticed the radio screen was black and stayed black(before this time, the screen would some times go blank or completely black and then turn back on). Car would start and shake really hard with noise coming from engine, and then car would die. Since then I have been going back and forth with Hyundai Motors and Hyundai dealer, telling them that this is not a warranty issue, but a safety issue and they both have ignored and didn't enology this at all. This happened October 4th, 2025 Recall of 182 was completed but it failed. The vehicle is at the Dalton Hyundai dealer in Nation City. Dealer confirmed zero compression in engine and want $5000 to tare engine to see if it would be under warranty but I keep telling them this is a safety issue that is still accruing and under the 573 Recall/19v-204 I tried to get help throw the BBB and got denied because of age and being the second owner and they told me to contact NHTSA I am a single mother of two young kids, I bought car on 01/26/25 and still owe money on car. Since 10/4/25 I have had to pay out of pocket for a car rental just get to work. My car is up for repo due to me not able to pay for a rental and my monthly car payment. I am lost and don't know what else I can do to get this matter seen. When buying this car the sale person said it is a good car, it will save money on gas, and it has been a nightmare.

Manufacturer Name: Tesla, Inc.

Vehicle Make: TESLA

Vehicle Model: MODEL Y

Model Year: 2023

Summary

The vehicle display flashed red and then the vehicle turned off completely while in traffic with no ability to change gear, operate the electronic door releases, or turn the hazards on. Safety was put at risk because insufficient notice of failure was given so it was impossible to get out of traffic, the vehicle could not be put in neutral so could not be pushed out of traffic, further the hazard lights could not be turned on so I had to stand in traffic waiving people around the vehicle. Because the electronic door releases didn't work, the doors could not be opened from the outside, had a front window not been open, it would have been easy to trap our child in the car. The dealer confirmed that the failure took place from vehicle logs but was unable to reproduce the problem. The dealer/manufacturer inspected the vehicle. Prior to this happening the vehicle showed warnings that power was reduced and vehicle may not restart about 20 minutes prior to the event, but the warnings disappeared after a minute. Even though Tesla confirmed the failure, they refused to diagnose the problem or fix it so the vehicle remains dangerous.

Manufacturer Name: General Motors, LLC

Vehicle Make: HUMMER

Vehicle Model: H3

Model Year: 2009

Summary

Category: Recall Reimbursement Problem NHTSA Complaint Description I am filing a complaint regarding General Motors’ refusal to reimburse an out-of-pocket repair related to Safety Recall N150042 on my 2009 Hummer H3 (VIN: [XXX] ). On December 31, 2025, I paid $946.06 out-of-pocket to correct the defect covered by Recall N150042 (blower motor connector module). The repair was performed at Coleman Buick GMC Cadillac in Lawrenceville, New Jersey. A complete reimbursement package was submitted to GM. GM has denied reimbursement, claiming that this recall was previously repaired in 2016. However: • No documentation has been provided to support that claim.
• No repair order number has been provided.
• No servicing dealer or repair location has been identified.
• No proof of recall closure documentation has been produced.
• No warranty claim record has been provided. Despite repeated requests, GM has refused to provide any evidence that Recall N150042 was completed in 2016. The defect was present in 2025 and was corrected by a GM-authorized dealership. If the recall had truly been completed in 2016, documentation should exist and be readily producible. GM is refusing reimbursement of $946.06 without providing supporting documentation of prior recall completion. This appears to involve either improper recall recordkeeping, improper VIN recall closure, or failure to comply with federal recall reimbursement requirements under 49 U.S.C. § 30120. I respectfully request NHTSA review of GM’s recall documentation and reimbursement practices related to Recall N150042. After submission, download or screenshot your confirmation number for your escalation packet. If you would like, I can now prepare: * A shortened version (if the online form has character limits) * A legally intensified version * Or a version framed specifically to trigger recordkeeping review rather than just reimbursement review INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
