Summary

I am submitting a formal safety complaint regarding a severe design defect in the 3.5L V6 engine of my 2013 Ford Explorer. The vehicle is equipped with an internal, chain-driven water pump that is currently failing at 100000 miles, as evidenced by coolant actively draining from the factory weep hole. Because this pump is located inside the engine block and driven by the timing chain, a complete seal failure dumps engine coolant directly into the oil pan. This is a critical safety hazard. When the coolant and oil mix, it destroys the engine bearings and causes the engine to completely seize without warning. If this sudden engine seizure occurs while driving at highway speeds, it results in an immediate and total loss of motive power, power steering, and power brakes. This defect creates an extreme crash hazard for the occupants of the vehicle and other drivers on the road. The internal weep hole provides virtually no dashboard warning or indicator to the driver before this catastrophic failure occurs. I am requesting that the NHTSA investigate this known, dangerous design flaw to force a mandatory safety recall before this sudden loss of vehicle control causes severe injuries or fatalities.

Summary

My truck experienced a P0302 misfire on the freeway, which caused it to shake immediately and made it very difficult to drive. I had it towed to the dealership, and they told me there was water in the spark‑plug wells, which was surprising since I live in Las Vegas, NV, where it rarely rains. My spark plugs were rusted as well as the cylinder head. After looking into it online, I discovered that Ford issued TSB‑23‑2011 for the Ford Ranger due to a windshield cowl leak that allows water to drip directly onto the engine. Their proposed fix is to install an engine cover, but that clearly doesn’t solve the issue since my truck already has the engine cover installed. Now I’m stuck paying for repairs caused by a design flaw that Ford never properly addressed.

Summary

My 2026MY Ford Escape is having intermittent rear view camera issues. On two separate occasions in the last week, when I put the vehicle into reverse, there is no image displayed on the center stack screen. The screen was completely blank and did not show what is behind my vehicle. I tried putting the vehicle back into park and then back into reverse and it still did not work. Yesterday, I took the vehicle to my local Ford dealership and was told that there is no recall for this. I worked at Ford for 32 years and I am familiar with the laws governing safety features on passenger vehicles. I am concerned that this is similar issue that was announced recently by Ford on the Bronco Sport. The technician at the Ford dealership said for me to keep driving it until Ford becomes aware of the issue specific to other 2026MY Escapes and then issues a recall and a potential fix. Based on my experience working for the company, this may take several months. I am concerned that this will lead to accidents that could endanger kids, pets, or personal property. Please compel the company to address this quickly. Thanks.

Summary

The contact owns a 2023 Chevrolet Traverse. The contact stated that while driving at various speeds, the vehicle unintendedly shut off, the instrument panel went black, and the vehicle lost motive power, and the vehicle lost power steering and power brakes functionality. The vehicle was shifted to neutral(N) and restarted, and regained power, and the vehicle returned to normal functionality. The failure also occurred while depressing the brake pedal. There were no warning lights illuminated. The failure had become more frequent and occurred approximately 3 - 4 times a day. The vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 20,000.

Summary

On February 21st at approximately 9:30 AM, I was driving my brand-new 2026 Chevy Suburban High Country when I began hearing an unusual noise from the engine. There were zero dashboard warnings — no check engine light, no alerts, nothing indicating a failure. Out of caution, I immediately headed to the dealership so they could inspect it. I was driving slowly and carefully when, without any warning whatsoever, the engine suddenly and completely seized. The vehicle locked up abruptly. I barely had enough time to try to move toward the shoulder, and I was unable to fully clear the roadway — the rear half of the vehicle remained partially in traffic. It was an extremely dangerous situation. What is especially alarming is that I was not on the highway at the time. Had this occurred while traveling 80–85 mph on SH-130 in Texas — which is a normal speed limit there — the outcome could have been catastrophic. A complete engine seizure at highway speeds could have caused a major accident involving multiple vehicles. Even more concerning, my Chevrolet app (connected through OnStar) still shows no issues or alerts with the vehicle. The attached pictures are from the My Chevrolet app on Sunday, 02/22/2026, around 11:13 AM. This is a brand-new vehicle, and I was informed yesterday that the dealership has requested a full engine replacement. I have no timeline for the repair. For a vehicle of this price point, a sudden engine seizure with zero warnings is deeply concerning from both a safety and reliability standpoint.
