Summary

Cold side intercooler boost tube cracked due to constant vibration impact from coolant overflow hose routed directly above and on top of it. Part and vehicle is available for inspection. The split in the boost hose caused me to loose power while attempting to merge onto a road, almost being hit by another vehicle. After research there are multiple other instances of complaints of the same thing happening to others and multiple aftermarket manufacturers producing replacement and upgraded parts with references and warnings of the same thing happening for the last 10 years. No warning lamps or symptoms prior to failure. After failure no warning lamps but could hear an audible hiss. The vehicle has not yet been inspected by the dealer or police. I was told by their parts people it is a regular failure and the part costs typically around $200 and they only sold it as a full part and not just the rubber replacement hose althought they are separate parts connected with hose clamps.

Summary

On February 19, 2026, I contacted Hendrick Toyota Apex (1210 Laura Village Rd, Apex, NC 27523) to schedule repair for two open safety recalls on my 2000 Toyota Tundra (VIN: [XXX] ) under NHTSA Campaign #09V444, Toyota Campaigns BXD and BZD. The dealership incorrectly refused to perform either recall, stating that neither could be done in North Carolina because they only apply to cold climate states. This is incorrect — Campaign BZD was specifically created as a supplemental campaign to allow Toyota dealers outside the 20 cold climate states to inspect and repair covered vehicles. I was turned away on my first call. Only after calling back a second time to lodge a complaint did a different representative look up the recalls and confirm that Campaign BZD can in fact be performed at their location. Had I accepted the initial refusal, I would have been denied a free safety recall repair on a vehicle with 255,656 miles and visible frame corrosion. The dealership's staff appear to be untrained on recall Campaign BZD and are likely turning away other affected Tundra owners. I have call recordings documenting the incorrect refusal. I am requesting NHTSA review this dealership's handling of recall #09V444 to ensure other consumers are not wrongly denied safety recall service. Summary

This submission is an additional report related to NHTSA complaint #11714381. At approximately 55,000 miles, my 2021 Chevrolet Blazer experienced a sudden throttle body failure while driving. The vehicle entered reduced engine power/limp mode without warning and acceleration was significantly limited. This created a safety concern while driving in traffic, particularly when merging and crossing intersections. Diagnostic code P2101 (Throttle Actuator Position Performance) was present. A Chevrolet dealer confirmed an internal throttle body failure and replaced the throttle body assembly. Although the repair was completed, this failure occurred at relatively low mileage for a critical engine control component. A sudden loss of acceleration can pose a safety risk depending on traffic conditions. Since filing my original complaint, I have identified multiple similar reports online and in owner forums describing premature throttle body failures in GM vehicles with similar symptoms (reduced power/limp mode and limited acceleration). This suggests the possibility of a broader pattern involving this component. I am submitting this additional report so that NHTSA has a complete record of the incident and potential pattern of premature throttle body failures. If this issue is widespread, it may warrant further review or investigation as it affects vehicle acceleration and safe operation.
